Nursing: The discipline of safe care

Nursing is the applied science of preventing harm where illness is already in the room.

The nursing process

Assess, diagnose, plan, implement, evaluate. Nurses move through this loop every shift for every patient, and the loop never skips assessment: if the data you gather is wrong, everything built on it inherits the error.

Assess → Diagnose → Plan → Implement → Evaluate

Safety first

Most nursing errors are systems errors, not knowledge gaps. The five rights of medication administration (patient, drug, dose, route, time) exist because each one has failed somewhere and hurt someone.

5 rights: patient · drug · dose · route · time
